Introduction: One of the main objectives of MMICS (the Multinational Musculoskeletal Inception Cohort Study), set up in forum VI was the inclusion of health-care factors as predictors of poor outcome. The collaboration researched and achieved consensus on which factors to target, both in terms of health-care and social system factors, first amongst ten national teams, and then through consultation with a panel of experts. The process involved developing a practical coding system which can be used in prospective cohorts, amongst other research studies. A primary goal (and obstacle) was the development of a coding system that could be applied across different health systems.

Objective: To disseminate and discuss the potential risk factors associated with the health-care and social systems in the prognosis of back care.

Methods / content: Background presentation of the MMICS project, its aims, method and current status (TP). 15 minutes. Focus on health and social care systems- why do they matter? (KB) 15 minutes. Measuring across different health care system- the pit of despair (MU) 20 minutes. Reaching consensus – the MMICS protocol for measuring health and social care factors (TP) 15 minutes Discussion of remaining problems, ideas for dissemination, collaboration and data collection- (Chair-AB).
